FAA is proposing $110,000 civil penalty against Air Evac EMS, Inc. of West Plains, MO for operating Bell BHT 206 helicopter that was not in compliance with Federal Aviation Regulations. Allegedly, chin bubble window was installed by company mechanic without following manufacturer's instructions. This installation was not documented in aircraft's maintenance logbook.
