When consumers affected by faulty ignition switches began filing claims against General Motors on August 1 , there were only 12 deaths linked to the malfunctioning parts.

Storms blamed for 2 deaths sweep across the South

A severe storm system roughed up much of the South for a second day Tuesday, knocking out power to tens of thousands as it snarled morning commutes and kept many under a tornado watch. The vast system, which claimed two lives Monday after spinning off tornadoes in Arkansas and Missouri, sent heavy thunderstorms across much of Georgia.

A handful of states are reconsidering a certain brand of highway guardrails after investigations revealed they were harming drivers instead of protecting them. Trinity Industries, the manufacturer of the highway guardrails, reportedly made a change to its design in 2005, one that allegedly can cause the guardrail to impale an impacting vehicle rather than curl out of its way.

UK - The Animal Health Veterinary Laboratories Agency (AHVLA) has reported neonatal deaths due to clostridial enterotoxaemia, the first outbreak of Klebsiella pneumoniae septicaemia outside East Anglia, polyuria, polydipsia and inappetance in adult sows due to vitamin D toxicity in its July 2014 Surveillance report.
